CD-ROM not mounting on Origin 2000

Hi there,
I'm trying to install IRIX 6.5 on an Origin 2000 deskside, this should
be very straight forward, but I'm being bugged by the CD-ROM...
The first hurdle came when the original CD-ROM started to act up, i.e.
would not close, but eject itself automatically. I know this is
potentially a simple super glue job, but I decided to go another route:
get a new sys controller+CD module to replace the one I had.
This module is newer than the one I have, i.e. the CD-ROM is of a newer
kind (XM-5701 vs. XM-5401), and I'm wondering if this is not the cause
of all this trouble that I am experiencing now: I am unable to mount any
kind of CD, software install reports "media error" right away...
I booted the Origin from a 6.4 disk and am unable to mount any CDs
either, syslog shows constant retries (and failures) on the CD-ROM... I
also fx'd a root drive on the Origin from an external CD-ROM (just to
make sure something was at least working properly on the Origin), but
IRIX won't let me install from there, it wants to use the internal CD-ROM...
Someone on nekochan had a similar issue on Origin and fixed it by doing
a resetenv in the PROM monitor, but I don't have that kind of luck...
Any suggestion? I'm thinking it must be some kind of SCSI issue
