The O2 has a habit of eating it's pram settings now and then. Just issue a
"resetenv" from the PROM commandline and all should be right with the world.

just in case the drive had died.

My O2 wonąt boot!

I have an O2 that booted IRIX 6.2 just fine but I didnąt have any
passwords for it so I bought a copy of 6.5.11 and went through the
arduous process of installing it which appeared to go just fine.
During installation I did issue the mkfs command to initialize the hard
drive since I did not need or want any of the data on the drive. After
installation I was was able to login to the root account, I changed the
IP address to work on my network and rebooted. I then received the
following message upon bootup:

unable to execute n=pci(0)scsi(0)disk(2)rdisk(0)partition(Cool/r=sash: no
such device
Unable to load bootfile: no such device
Unable to boot; press any key to continue:

After pressing the return key, Iąm taken the the display page where I
can;

Start System
Install System Software
Run Diagnostics
Recover System
Enter Command Monitor
Select Keyboard Layout

Clicking Start System brings me to a similar message that reads:

unable to execute n=pci(0)scsi(0)disk(2)rdisk(0)partition(Cool/r=sash: no
such device
Unable to load bootfile: no such device
Autoboot failed
Unable to continue; press <enter> to return to the menu:

Returning to the menu, clicking Install System Software, selecting the
local scsi cdrom drive 4 option and clicking install results in an error
message that reads:

Unable to open łn=pci(0)scsi(0)disk(2)rdisk(0)partition(0)˛: no device

Clicking continue takes me back to the menu where I run diagnostics
which from what I can tell completes without problem.

If I select Recover System I also get the

Unable to open łn=pci(0)scsi(0)disk(2)rdisk(0)partition(0)˛: no device

error message. Upon entering the Command Monitor and typing hinv, it
lists 2 drives;

SCSI Disk: scsi(0)disk(2)
SCSI Disk: scsi(0)cdrom(4)

Typing printenv shows several options, including:

SystemPartition=n=pci(0)scsi(0)disk(2)rdisk(0)partition(Cool
OSLoadPartition=n=pci(0)scsi(0)disk(2)rdisk(0)partition(0)
OSLoadFilename=e==/unix
OSLoader=r=sash

So at this point I am kind of stuck. I canąt install the OS because it
canąt seem to find the hard drive yet it seems to know it is there.

Any ideas?
